膀胱過勞「尿失禁」是危險訊號

恩主公醫院泌尿科高銘鴻醫師表示,他曾收治一名80歲的伯伯,當他走進診間,空氣中瀰漫著一股淡淡的尿騷味,伯伯一直沉默不語,反倒是陪病的兒女緊張的說,爸爸有尿失禁問題。原來,近日疫情趨緩,孝順的兒子想帶父母出門走走,但旅程中伯伯不斷催促家人找廁所,下車後還來不及到洗手間就尿褲子了。伯伯回到家大發雷霆,責怪家人逼迫他出門,原來他平時就頻尿,有時還會漏尿,出門不方便,這次在大家面前出醜,讓伯伯感覺很受傷。

 

經問診後發現,原來伯伯有攝護腺肥大的困擾,先前診所開的藥吃了有稍微改善就停,又有症狀時再吃,前陣子因疫情嚴重就沒回診,也懶得吃藥,這次來院檢查後發現,攝護腺肥大狀況非常嚴重,已經堵塞整個膀胱出口,長久下來膀胱用力過度,造成嚴重增厚的狀態。

高銘鴻醫師指出,攝護腺肥大的症狀可分為排尿及儲尿症狀,前者會有排尿時需要用力、等一段時間才能排出,尿柱斷斷續續,尿到最後還會滴滴答答;後者則是頻尿、夜尿、尿急就憋不住,其中最危險的訊號是「尿失禁」。當攝護腺肥大造成膀胱出口嚴重阻塞後,膀胱的逼尿肌就要很用力才能排尿,長久下來膀胱逼尿肌就會「過勞」,排尿力量會大幅度下降,造成排尿愈來愈少,留在膀胱餘尿量愈來愈多,就會從尿道滲出來,也就是「急迫型尿失禁」。

壓力回流到腎臟 造成腎病變

醫師說明,當整個膀胱處於過勞狀態時,膀胱一直在儲存過量的尿液,無法自主排出,於是一陣一陣的漏尿,變成「滿溢型尿失禁」;而當膀胱壓力太大時,多餘的尿液會回流至腎臟,容易造成「水腎」,腎臟的壓力太大、過濾血液的功能下降,久而久之影響腎功能,最後可能變成末期腎病變而需要洗腎。

 

高銘鴻醫師建議,攝護腺肥大可視為像糖尿病、高血壓一樣的慢性病,是老化的現象之一,也是影響年長男性生活品質的重要一環,而治療方法就從保持良好生活習慣著手,此外針對各種下泌尿道症狀進行藥物治療,都可以有良好的效果,即便是藥物治療效果不佳的病人,目前無創的內視鏡電刀、雷射手術,都可以達到接近根除症狀的效果。

 

關心長輩令人難以啟齒的困擾,尿失禁通常是男性攝護腺肥大的末期症狀。高銘鴻醫師呼籲,如發現家中長者在廁所的時間變長、頻尿等狀況,都可以及早就醫治療,改善長輩的生活品質。
